Worked a shift that was 11 am to 3:45 pm and went over my fifth by 3 minutes because my team lead told me to get on the checkout lane and help guest when I was leaving in 5 minutes.

Yeah, it's bad. Sorry but you should've told your TL that you were just about to hit your fifth. One time I was about to hit my fifth when my TL told me to jump on the lanes. I told him sorry, I can't because I was just about to hit my fifth. He thanked me for telling him and told me go ahead and punch out.
Missing your 5th is serious...Usually starts as coaching, continuous you could be written up. Its a compliance thing to make it look like Target is giving you a well scheduled lunch.
Isn't this dependent on state law? In my state, we have to take our lunch before starting our sixth hour. I typically take my lunch at 5 1/2 and no one gets after me about it. (The break room is quieter and less crowded so I can read my book in peace.)

Because we have an HR ETL who gets twitchy about these things. They get twitchy about a lot of things but this is one of the few where there are real penalties possible because of state law.
When we bring on seasonal TMs, there are daily announcements over walkie about it.
Long time ago, before they finally figured out that this is a pattern for me and not an almost-violation, they used to nag me about it. Also over walkie.
