Archived Long term disability

Does anyone remember Target offering LTD to all employees in 2015 and no one would need a physical and pre existing conditions would not matter? I have talked with several fellow team members and they remember but no one has the paperwork anymore. And help appreciated.

Long term disability is offered as an option that you pay for. Short term disability is free & up to 90 days.

I don't remember any paperwork. Just signed up for it with benefits.

I also signed up last year. This year they are making me fill out all of the paperwork saying I don't already have it. Looking for the paperwork tomorrow.
