Archived supervisor?

ok this might be a weird question but I'm applying for a new job and its asking for a supervisor. i just wanted to know do i put a team leaders name an L O D name?

I'd put your TL name since they are the one who was your direct supervisor.

What commiecorvus said. If you have more than one TL over your workcenter, use whichever one you're aligned with (i.e. they do your review).
