Personal leave

For a long time, its store to store policy.

Of my three stores, time off restrictions varied year to year. No timeoff from Nov 1 or Thanksgiving to New Years. Some said get your time off in by mid Nov but nothing more than 1-2 days allowed per 2 week cycle.

I would think it depends on the Loa type. Medical like needing surgery or dealing with a serious illness yes to Loa. Mental health Loa possibly. Loa to take a trip to Japan for a month absolutely not. Yes someone at my store took a month Loa to go to Japan. It was not during 4th quarter.
 
A TM at my store is going on leave soon for surgery. (It's been planned for a while, but was delayed at least once.) Another has been on leave for a while, also for surgery; she was supposed to be back already, but developed a serious infection at the incision site and then her elderly dad became seriously ill so her leave has been extended.
 
I just came back from having surgery to remove lung cancer. Its gone and I need no treatment, but I am still having discomfort and dont have near the strenght I once had. I could retire as I am of age but like being able to work part time, I just cant handle the Q4 madness anymore, I will have a conversation and see what they can do to help me.

Seems like your circumstances would allow for some leeway. (Glad you're on the road to recovery!) Maybe reduce your hours or move to a different work center?

If they say no to a leave then, maybe consider going on demand. You could work when you want to and only have to work once every 6 weeks. Later after the holidays you could switch back to part time.
 
If you request FMLA (which you can do through the LeavePro portal via eHR), nobody at your store can deny it to you, Q4 or not. Decisions regarding FMLA leave are handled by an outside team. If you can get your doctor to fill out some paperwork attesting you need time off work for your recovery, you'll be good.
