Archived Intermediate LOA?

I have been having SERIOUS mental health issues regarding panic attacks and bipolar disorder. I explained it to my STL, ETL, and some LOD's. They suggested taking an intermediate LOA,

I'm afraid that I wouldn't be approved and it's scary. It keeps getting worse at this point, the other day I had a REALLY bad panic attack AT work, I asked to go home early and ended up having another one about 2-3 hours later.

Has anyone been in at least a similar situation? How do I go about the process?

If your leadership is suggesting an LOA, it should be approved since they could back up your reasons.
Talk to your HR-ETL asap.

Go on a leave. Get a note from your DR if you can. I had a tm with similar issues, and he went on a full leave right away. After things were getting better, he came back with some restrictions. Really shouldn't be a problem.
