Target Best Health Rewards Program Overview

Eligible team members who are enrolled in a Target-sponsored medical plan and participate in the Target Best Health Rewards program can earn rewards by completing the following activities:

•$25 Target GiftCard® by completing a confidential Health Assessment between 4/1/2012 and 9/30/2012.
•$25 Target GiftCard® by entering their biometric results between 10/1/2010 and 9/30/2012.
An enrolled spouse or domestic partner can also earn the rewards if they complete the above activities.

If the Health Assessment and biometric screening are both completed, the family will receive one entry into a sweepstakes for free 2013 medical premiums (a value of up to $6,000).
